What “Vagal Tone” Means, and Why HRV Is a Proxy, Not a Direct Measurement

You cannot measure vagal activity directly outside a lab with implanted electrodes. Instead, researchers rely on heart rate variability, the small, natural variation in time between consecutive heartbeats.

Higher HRV during calm breathing generally reflects stronger parasympathetic, vagally-mediated influence on the heart. That is a reasonable proxy, but it is still a proxy. HRV is also affected by age, fitness level, hydration, medication, caffeine, and even the specific measurement window used.

This distinction matters for reading claims about vagal tone. A technique that raises HRV for the ten minutes you are doing it is not automatically proof of a lasting change in nervous system function, mood, or resilience. Short-term physiological shifts and long-term mental health outcomes are two different questions, and most of the available research only answers the first one.

Techniques With Real Evidence Behind Them

A few low-tech techniques show up repeatedly in controlled studies measuring HRV. None of them require equipment, and the effects are generally measured within a single session.

Slow Diaphragmatic Breathing

Slowing the breath to around six breaths per minute, roughly a five-second inhale and five-second exhale, is one of the better-studied vagal techniques. A 2021 study published in the International Journal of Environmental Research and Public Health tested single sessions of slow-paced breathing at six cycles per minute in 59 participants, at durations ranging from 5 to 20 minutes.

RMSSD, a common HRV marker of vagal activity, increased in every slow-breathing condition compared to a resting control. Interestingly, longer sessions did not produce a bigger effect than shorter ones, there was no clear dose-response relationship for vagal activity itself, though longer sessions did lower breathing rate further during the recovery period afterward.

Cold-Water Face Immersion (the Mammalian Dive Reflex)

Briefly immersing the face in cold water triggers the mammalian dive reflex, a reflex arc involving the trigeminal nerve in the face and the vagus nerve, which produces a drop in heart rate.

A 2023 systematic review and meta-analysis in the journal Psychophysiology pooled 17 studies and 311 participants examining the diving response. It found a moderate-to-large increase in vagally-mediated HRV during exposure to diving triggers such as face immersion and cooling. Total body immersion produced a larger effect than forehead cooling alone.

The authors were candid about limitations: the effect largely disappeared after exposure ended, the studies used inconsistent methods and small samples, and they rated the overall body of evidence as low quality. This is a real, physiologically well-understood reflex, but the research base behind it is thinner than the confident tone of most wellness articles would suggest.

Humming and Gargling

Humming and gargling both mechanically engage muscles and nerve branches connected to the vagus nerve in the throat and soft palate. A small 2023 pilot study in the journal Cureus fitted 23 participants with a wearable Holter monitor and compared HRV during humming (a simple Bhramari pranayama exercise), physical activity, emotional stress, and sleep.

Humming produced the lowest stress index of the four conditions and higher HRV markers than physical activity or stress. The authors were upfront that their small sample size limits how far the findings can be generalized.

A separate, older pilot fMRI study of chanting “Om” in 12 volunteers found reduced activity in brain regions like the amygdala, similar to patterns seen with clinical vagus nerve stimulation devices. The authors of that study explicitly described their vagal explanation as preliminary and said they could not rule out that the effect was simply a general relaxation response rather than a vagus-specific one.

Consumer vagus nerve stimulation devices marketed for anxiety and mood generally have far less independent research behind them than the free techniques above, and much of what does exist comes from manufacturer-funded studies with small samples. Ice baths and cold plunges are often described as vagal tone tools, but most of the direct HRV evidence is specific to face immersion, not full-body cold exposure at length.

Essential oils, specific “vagus nerve” supplements, and single-session “trauma release” claims tied to vagal tone are not supported by the kind of controlled physiological research described above.

Why the Evidence Is Weaker Than It Sounds Online

It is worth being honest about the quality of the underlying science, since wellness content rarely mentions it.

Sample sizes are small. The studies cited above range from 12 to 59 participants directly, and even the 17-study meta-analysis only covered 311 people total across very different protocols. That is normal for early physiology research, but it means findings can shift as larger studies are done.

HRV is a correlate, not a direct readout of mental state. A technique can reliably raise HRV during a lab session without that translating into measurably less anxiety or better long-term stress resilience, since most of these studies measure heart rhythm, not mood or clinical outcomes, over a single sitting.

Self-report and expectation effects are hard to rule out. When a study asks people to rate how calm they feel after a breathing or humming exercise, expecting the technique to work can itself shift the answer, independent of any physiological change.

Correlation and causation get blurred. Higher resting HRV is associated with better stress regulation in population studies, but that does not automatically mean any single technique that nudges HRV up for a few minutes will produce the same benefits as naturally higher baseline HRV.

How to Use These Techniques Sensibly

None of this means the techniques are worthless, it means they are best treated as a low-cost, low-risk part of a stress-management routine rather than a treatment.
