Where the Creatine-Hair-Loss Concern Actually Comes From
The entire creatine-hair-loss claim traces back to one study: a 2009 double-blind, placebo-controlled crossover trial published in the Clinical Journal of Sport Medicine, led by van der Merwe and colleagues, on 20 college-aged rugby players in South Africa. After a loading phase (roughly 25g/day for 7 days) followed by a maintenance phase (5g/day for 14 days), the creatine group’s serum DHT rose about 56% after loading and stayed roughly 40% above baseline through maintenance, while total testosterone itself did not change significantly. The DHT-to-testosterone ratio also increased. The authors’ own conclusion was narrow: creatine may increase the rate at which testosterone converts to DHT, and they called for more research into long-term safety. They did not photograph, count, or otherwise assess a single hair follicle.
Why DHT Gets Blamed for Hair Loss in the First Place
DHT is a byproduct of testosterone, converted by the enzyme 5-alpha reductase. In people who are genetically predisposed to androgenetic alopecia (pattern baldness), hair follicles on the scalp are unusually sensitive to DHT, which gradually shrinks them in a process called miniaturization — this is the same pathway drugs like finasteride target by blocking DHT production. So Does Creatine Actually Cause Hair Loss?
The honest answer is that there is no direct evidence creatine causes hair loss, and the best available direct evidence (2025) points the other way. The original concern was never about hair at all — it was about a single small study showing a hormone shift in a hormone that’s associated with hair loss in genetically susceptible people, and that link had gone untested against real hair outcomes for over a decade.
