Why Creatine Deserves a Second Look After 40

Creatine has spent decades marketed almost entirely to young male athletes chasing bigger lifts, which left a lot of women assuming it wasn’t for them, or worse, that it was actively working against the changes they wanted. Neither is true. A 2023 review in the Journal of the International Society of Sports Nutrition covering creatine across a woman’s lifespan notes that women naturally carry 70 to 80 percent lower stores of creatine in their bodies than men do, largely a function of typically smaller muscle mass and different dietary intake patterns. That gap doesn’t shrink the case for supplementing, it strengthens it. If your baseline reserves are already lower, a modest daily dose has more room to make a measurable difference.

The Bloating and Bulking Myths, Debunked

The two hesitations that come up most with women are also the two most misunderstood parts of how creatine works. The first is water retention. The same lifespan review linked above describes the early weight increase seen with a creatine loading dose as reflecting increased cellular hydration, water pulled into muscle tissue itself, and frames it as a functional, even a positive, effect rather than a cosmetic problem. It is not the kind of surface bloating people picture, and it typically settles once the loading phase ends.

The second worry, that creatine will make a woman look bulky, conflates two very different processes. Visible bulk comes from years of progressive resistance training building muscle tissue, not from a supplement. Creatine’s actual job is to help muscle cells regenerate energy faster during that training, which supports slightly better performance and recovery, it does not manufacture muscle out of nothing. The research reviewers behind that same paper describe concerns about unwanted weight gain in women as largely unfounded.

What Happens to Creatine Around Perimenopause and Menopause

The hormonal shift of perimenopause and menopause is precisely why this topic matters more at 40-plus than it did at 25. The same 2023 review found that post-menopausal women may see the clearest benefit in skeletal muscle size and function from creatine, particularly at higher intakes in the range of roughly 0.3 grams per kilogram of bodyweight daily, which is the short-term loading-style dose rather than a maintenance amount. The authors also point to lower creatine concentrations in the female brain, especially the frontal lobe, as a reason supplementation may carry a meaningful cognitive upside for women specifically, a detail almost never mentioned in general, male-oriented creatine content.

What the Research Actually Shows

Muscle and Strength

A meta-analysis published in Nutrients, pooling 10 randomized controlled trials with 211 older women, found creatine combined with resistance training produced a significant increase in upper-body strength overall, and a significant lower-body strength benefit specifically in trials that ran 24 weeks or longer. Shorter interventions didn’t show the same effect, which is a useful reminder that this is a slow-build supplement, not a quick fix. That review did not find a measurable effect on muscle mass itself independent of duration, even though strength improved, a distinction worth keeping in mind since strength and muscle size don’t always move together in the research.

A newer and larger pooled analysis, described in a 2025 systematic review and meta-analysis covering 7 trials and 608 postmenopausal women, found a small but real lean-mass gain of about 0.37 kilograms and a leg-press strength improvement of roughly 7.5 kilograms, with the clearest benefits showing up when creatine intake was at least 5 grams a day and paired with resistance training. Lower doses without training didn’t move the needle. Together, these two reviews paint a consistent picture: creatine helps, but training is doing real work here too, not the supplement alone.

Bone Density: A More Complicated Picture

Bone health is the one area where the evidence is genuinely more mixed, and it’s worth saying plainly rather than glossing over. A rigorous 2-year, placebo-controlled trial in postmenopausal women with osteopenia, published in Medicine & Science in Sports & Exercise, found that 3 grams of creatine daily did not improve bone mineral density at the spine, hip, or femoral neck compared with placebo, and had no meaningful effect on bone microarchitecture, biochemical markers, or muscle strength over that period. The larger 2025 meta-analysis mentioned above reached a similar conclusion, reporting bone density unchanged overall across the pooled trials. If bone density specifically is your main goal, creatine on its own isn’t the evidence-backed answer, weight-bearing and resistance training remain the better-supported tools there.

Cognitive and Mood Effects

Creatine isn’t only a muscle-and-bone story. The lifespan review noted above points out that women tend to have lower baseline creatine levels in brain tissue, and suggests supplementation may support a more energetic state in brain cells during hormonally turbulent years, when both mood and mental sharpness can take a hit. This is an emerging area of research rather than a settled one, but it’s a genuine reason the case for creatine in women over 40 goes beyond the weight room.

How Much to Take: Dosing for Women

Dosing guidance for women largely mirrors what’s used in the general creatine research, adjusted for the fact that many women weigh less than the men most older studies were built around. A few practical starting points drawn from the trials above:

  • Standard maintenance dose: 3 to 5 grams of creatine monohydrate daily, taken consistently, is the amount used in most of the postmenopausal trials showing benefit.
  • Skip the loading phase if you prefer. A short loading period (around 20 grams a day split into doses for 5 to 7 days) saturates muscle stores faster, but a steady 3 to 5 grams daily gets you to the same saturation point within roughly three to four weeks.
  • Timing doesn’t matter much. Total daily intake and consistency drive the benefit far more than taking it before or after a workout.

The meta-analysis of older women specifically reported no dropouts attributable to side effects.
